DESCRIPTION:Monday - Friday 8am-5pm (no weekends or overtime), working primarily from the office!
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Assesses, plans, implements, coordinates, monitors, evaluates options/interventions and develops a comprehensive plan of care for the patients.
Promotes the achievement of optimal clinical and resource outcome.
Performs Care Management Function in the practice, participates in a primary care physician led team that takes collective responsibility for the continuum of patient care for a medium- high and high risk population.
Performs ongoing communication and works to promote quality of care through collaboration with PCP, specialist and all service team members, patients and families.
Is responsible for identifying, assessing, planning, coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load.
Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services.
