A very well-known hospital group, located in the heart of San Francisco is searching for a talented RN Case Manager for their hospital.

This is a 40 bed specialty hospital that has the unique balance of a culture that has the close-knit and passion of a small community hospital, but the resources and financial security of a mid-sized health system with 20+ locations.

This role is ideal for someone who wants a casual atmosphere and working environment, looking to stay away from the corporate buzz and jargon that tends to arise in larger corporations. **M-F/8-5 (flexible) schedule**

RESPONSIBILITIES:

• Coordinate clinically complex patient’s care across a continuum; ensuring and facilitating the achievement of quality clinical and cost outcomes.
• Negotiating, procuring and coordinating appropriate services and resources needed by the patients.
• Ensures appropriate sequencing of treatment goals and implementation of a comprehensive discharge plan.
• Demonstrate knowledge and skills necessary to provide case management services to the patient population experiencing rehabilitation needs.

JOB REQUIREMENTS:

• Registered Nurse RN
• 1+ experience within Case Management in a Hospital setting (Case Manager, Care Manager, Case Management, Care Management, Utilization Review, Utilization Management, Care Coordination, etc.)
• 2+ years working in an inpatient / acute care setting- Home Health or Hospice will not be accepted unfortunately
