I'm partnered with a Non-Profit to find a Full-Time Physician to join a PACE program in Brighton, MA!

As part of the interdisciplinary team, you’ll collaborate closely with nurses, social workers, therapists, and other healthcare professionals to provide holistic and patient-centered care. This position offers you support through your team and the opportunity of fulfillment delivering quality geriatric care.

Responsibilities:

  • Completes initial, periodic and annual physical examinations of the participants.
  • Evaluates participants for episodic visits, for acute illnesses and for follow-ups of hospitalizations in the clinic, at home or at a nursing home.
  • Makes nursing home visits in accordance with the state nursing home requirements and as needed.
  • Use telehealth as needed and where appropriate
  • Participates in the PACE hospital rounding system and the on-call schedule.
  • Evaluates the needs of the participants and develops treatment plans in concert with the other members of the Interdisciplinary Team.
  • Perform other duties as required.

Qualifications:

  • Board Certified in Internal Medicine or Family Practice, or eligibility for Board Certification (for recent graduates).
  • Board Certified in Geriatric Medicine, preferred.
  • Current License to practice medicine in the Commonwealth of Massachusetts.
