No experience is required, but we prefer someone with case management experience. You also need to be fluent in Spanish!
Responsibilities:
Assess patient needs and develop care plans
Coordinate care with other healthcare providers
Facilitate communication between patients, families, and providers
Monitor patient progress and make adjustments to care plans as needed
Advocate for patients and their rights
Utilize EPIC EMR
Qualifications:
Registered Nurse license
One year of experience in case management (preferred but not required!)
