Housing Case Manager

Are you looking for a rewarding opportunity to make a positive impact in San Francisco? AMI Healthcare Staffing, part of AMI Network, has a 2-month temporary contract with a possible extension for a Housing Case Manager with a leading non-profit organization in SF. As a Housing Case Manager, you will provide case management services to San Francisco's homeless population transitioning into housing. You will also conduct intake, match, and refer guests to housing services, assist guests with completing housing onboarding paperwork, link guests to appropriate resources, and develop and reassess Housing Stability Plans for guests every three months. You will maintain regular contact with guests, track their progress, and enter pertinent data relevant to guests.

To be considered for this role:

  • You must have excellent communication and interpersonal skills, be able to work occasional evenings and weekends, and have experience working with substance use disorders, mentally ill, and dually diagnosed clients.
  • You should also possess a working knowledge of and sensitivity to individuals with substance abuse and/or mental health issues such as PTSD, psychosis, borderline behaviors, bipolar, etc.
  • Intermediate computer knowledge is necessary as you will use Epic, ONE System, and other systems daily.

To succeed in this role:

  • You should have experience working with the homeless population, possess critical thinking and problem-solving skills, have excellent decision-making capabilities, and a collaborative work style.

The physical demands of this role include:

  • Frequent sitting, occasional walking and standing, and the ability to hear conversations in quiet environments.
  • Irregular or occasional work hours in the community are also expected..
