We are hiring two LVNs to join our leading non-profit client working with the unhoused population! This will be based at their recuperative care site in Glendale, CA. This is a 40 hour a week schedule, Monday through Thursday from 9am to 5:30pm or Tuesday through Saturday from 9am to 5:30pm.
As an LVN Medical Coordinator, you will be responsible for providing medical care to guests, including:
Facilitating the hospital discharge process
Conducting medical assessments and creating discharge plans
Providing education on medications, preventative measures, and general health
Coordinating follow-up medical appointments
Participating in case management team meetings
Performing administrative tasks such as charting and answering phones
We are looking for an LVN who is:
Licensed and in good standing
Up-to-date on TB testing and immunizations
Actively certified in BLS
Experienced working with homeless patients (preferred)
Able to work under pressure and stay calm in stressful situations
Comfortable working with patients from diverse backgrounds
Excellent communicator with strong interpersonal skills
Proficient in Microsoft Office Suite
Bilingual in another language commonly spoken in Los Angeles County (preferred)
