About Us: Our leading nonprofit healthcare client in San Diego, CA is a non-profit organization committed to serving our community. We are excited to announce the opening of three Case Manager positions at one of their clinics, where the focus is on substance use disorder (SUD) treatment and recovery.
Job Overview: We are seeking three dedicated Case Managers to join the team. In this role, you will play a crucial part in helping clients recover from substance use and co-occurring disorders. Your support, including role modeling and trauma-informed care, will guide clients toward addiction treatment, community resources, transportation, and other essential services.
Key Responsibilities:
Assist clients in their journey to recovery from substance use and co-occurring disorders.
Link clients to addiction treatment, community resources, transportation, and other necessary services.
Provide empathic, trauma-informed support as a part of the recovery process.
Collaborate with clients to develop and implement individualized care plans.
Offer crisis response and access to emergency care when needed.
Establish and maintain professional relationships with community providers and referral sources.
Provide transportation to clients for appointments and services when required.
Qualifications:
Bachelor’s degree or higher in social work, behavioral sciences, psychology, or a related field.
One year of progressively responsible experience in a SUD setting is preferred.
Certified SUD Counselor or Licensed Practitioner of the Healing Arts (or License Eligible Practitioner of the Healing Hearts).
Valid California Driver’s License with current automobile insurance.
