As a Social Services Coordinator, you'll provide post-hospital care services to guests experiencing homelessness as they transition out of an acute care hospital. Your role involves assessing needs, coordinating services, and ensuring program objectives are met. You'll also serve as a community and organizational resource on healthcare and housing issues.
Key Responsibilities
Complete Needs Assessment and Care Plans.
Facilitate the discharge process and locate housing for individuals.
Maintain an up-to-date database and partner contacts.
Attend care coordination meetings and collaborate with the nursing team for weekly case conferencing.
Practice harm reduction principles and support appropriate guest interventions.
Engage and conduct daily check-ins with guests to build rapport and trust.
Manage a caseload of a minimum of 15 guests with flexibility to meet facility needs.
Complete additional tasks as assigned to meet facility requirements.
Requirements
Bachelor’s Degree Required; Master’s in social work preferred.
Minimum of three (3) years relevant experience in healthcare delivery, hospital education, social services, or a similar field.
Excellent oral and written communication skills.
Positive and professional attitude.
Strong time management and organizational skills.
Proficiency in MS Word, Excel, Outlook, PowerPoint, and typing.

This role is non-exempt and requires on-site presence in Ventura.
Your role may require frequent standing, walking, sitting, bending, and lifting.
