Job Overview:

Key Responsibilities:

  • Assist with the hospital discharge process and patient assessments.
  • Create medical care plans for our guests.
  • Keep guest charts up to date using our CRM database.
  • Conduct medical and wellness checks, educate guests on medication regimens, and follow up on medical insurance.
  • Coordinate and schedule guest medical appointments.
  • Collaborate with our Intake team to ensure eligibility for recuperative services.
  • Participate in various meetings and trainings.
  • Advocate for our guests and follow the instructions of the LVN Manager.
  • Manage a flexible caseload to meet facility needs.
  • Track and update guest information in our database.
  • Handle administrative tasks like charting and answering phones.
  • Be flexible and assist with any task necessary to support our guests.

Qualifications:

  • LVN License or a bachelor's degree in Nursing.
  • Current American Heart Association BLS (CPR/First Aid/AED).
  • Up-to-date TB test and immunizations.
  • Prior experience with homeless patients (preferred).
  • Spanish language skills (preferred).
  • Ability to stay calm under pressure.
  • Experience with substance abuse and diverse patient backgrounds.
  • Reliable transportation and current auto insurance.
  • Effective communication and social skills.
  • Exceptional organizational abilities.
  • Proficiency in MS Word, Excel, Outlook, and typing.

Physical Requirements:

  • Frequent standing, walking, and sitting.
  • Bending, squatting, reaching, twisting, and kneeling.
  • Lifting and moving up to 50 pounds.
  • Conversational speech for effective communication.
  • Ability to hear equipment alarms and verbal communication.
