We are seeking a compassionate and dedicated Social Worker to join our team and play a pivotal role in our VSSR (Virtual Social Support and Rehabilitation) pilot program. As a Social Worker in this role, you will be responsible for providing essential support, guidance, and intervention to individuals who are enrolled in the program across six different sites. Under the supervision of the program director, you will be actively involved in site visits and will contribute to initiatives such as suicide prevention and overall emotional well-being.

**Responsibilities:**

  • Establish and maintain positive working relationships with individuals enrolled in the VSSR pilot program, numbering up to 125 people.
  • Collaborate closely with the program director to execute the program's objectives effectively and provide regular updates on individual progress and challenges.
  • Conduct thorough assessments of each participant's needs, strengths, and challenges to develop personalized support plans.
  • Implement a variety of therapeutic techniques and interventions to address issues such as suicide prevention, emotional regulation, and overall mental health improvement.
  • Coordinate and facilitate group therapy sessions and workshops, promoting open communication and creating a safe and supportive environment.
  • Provide crisis intervention and support to participants in times of acute emotional distress or emergencies.
  • Regularly travel to the six program sites to meet with participants in person, build rapport, and provide direct assistance as needed.
  • Maintain accurate and up-to-date case notes, progress reports, and documentation in accordance with program standards and ethical guidelines.

**Qualifications:**

  • A master's degree in Social Work or a related field.
  • Registration as a licensed or registered associate Social Worker, in accordance with state requirements.
  • Experience in providing mental health support and interventions, preferably in areas such as suicide prevention.
  • Proficiency in assessing needs, developing care plans, and implementing evidence-based interventions.
  • Demonstrated understanding of ethical considerations in social work practice.
  • Willingness to travel to different sites as required for in-person meetings and interventions.
