Patient care manager – RN with hospice and managerial exp
- 1 year of managerial exp + at least 1 year of hospice experience in the past 5 years required
- Direct Reports: 4-5 RN Case Managers + 4 HHAs (field staff)
- Nurse manager will be responsible for a census of 60-80 (that will determine how many RN Case Managers + HHAs they will have under them)
- Salary: $120k (commensurate with their experience) – exempt position
- Potentially 107k-138k
- Certified as a hospice and palliative care Nurse is a plus – not mandatory but a HUGE PLUS
- Strongly encouraged to get hospice & palliative certified,
- Additional program duty – either admissions or afterhours – that will add a couple more staff members to their team
- On-site – can do hybrid 1 day a week (has to be a nurse 8:30-5pm)
- Administrator on-call rotation – resource person 5pm on Friday – 8:30am Saturday morning
- Be there as a resource for case managers working in the field after-hours (i.e. end-of-life)
