Position Overview:

As a Nurse Practitioner (NP) in San Francisco, you will be an essential part of a leading non-profit organization committed to serving low-income people of color in Bayview and throughout San Francisco. The non-profit focuses on building empowered, clean, safe, and healthy communities, addressing unemployment, homelessness, and advocating for basic human rights.

 

Key Responsibilities:

  1. Provide direct patient care, including comprehensive assessments, treatment planning, and ongoing management of chronic and acute health conditions.
  2. Collaborate with an interdisciplinary team to ensure comprehensive care for patients.
  3. Adhere to clinical guidelines and protocols established by the organization.
  4. Document patient encounters thoroughly and accurately in electronic health records (EHR).
  5. Participate in regular team meetings and clinical case reviews.
  6. Provide education and counseling to patients and their families on health promotion and disease prevention.
  7. Coordinate referrals to specialty care and community resources as needed.
  8. Support quality improvement initiatives and participate in data collection and reporting activities.

 

Qualifications:

  1. Completion of an accredited Nurse Practitioner or Physician Assistant program.
  2. Current licensure as an NP or PA in the state of California.
  3. At least 2 years of post-graduate experience as an NP or PA.
  4. BLS and ACLS certifications (preferred).
  5. Strong clinical assessment and diagnostic skills.
  6. Excellent communication and interpersonal skills.
  7. Ability to work independently and as part of a team.
