We have an FHQC client seeking a dedicated Family Nurse Practitioner/Physician Assistant to join our team on a contracted basis. In this role, you will work closely with our Chief Medical Officer to deliver comprehensive primary care services, including health promotion, disease prevention, diagnosis, treatment, and monitoring. As a key staff member, you will also serve as a community liaison and advocate, upholding the standards and guidelines within a holistic, patient-centered framework.
Schedule: M-F 8a-5p
Pay: $77-$111hr
Locums Contract - 3mo - ongoing.
Key Responsibilities:
- Assess patient's health status by reviewing medical histories, conducting screenings, and performing diagnostic tests.
- Assist the Chief Medical Officer in medical procedures.
- Develop and implement care plans for patients, monitoring their conditions and progress.
- Interpret and evaluate individuals' health care needs.
- Write prescriptions and provide patient education on disease prevention and wellness.
- Contribute to quality improvement initiatives at the Center.
- Maintain professional relationships with staff, clients, and the public.
- Adhere to policies and procedures for safe work practices, including patient confidentiality and risk management.
- Consult with the Chief Medical Officer as needed based on patient needs and assessment findings.
- Communicate effectively with other MCHWC providers, staff, and volunteers to ensure consistency and continuity of care.
Qualifications:
- Current license to practice as a Nurse Practitioner or Physician Assistant in California.
- Master's Degree in Nursing and National Certification from accredited schools.
- Experience working in an outpatient health clinic setting, preferably in a community health center with underserved populations.
- Valid DEA Certificate and current CPR (BLS) certifications.
- Proficiency with Electronic Medical Records.
