Title: Case Manager
Pay: $23 - 25.44/hr

Are you ready to make a difference? We’re looking for a dynamic Case Manager to join our nonprofit client in Los Angeles and help them support children, youth, adults, and families in accessing essential resources like health insurance, housing, and affordable child care. You’ll team up with therapists to conduct thorough needs assessments and provide individual and group rehabilitation services under various agency contracts.
Key Responsibilities:
Collaborate with therapists to assess needs and connect clients with vital resources.
Deliver individual and group rehabilitation services as per Department of Mental Health contracts.
Meet productivity standards for client service hours and ensure timely completion of progress notes.
Offer guidance to parents on effectively managing child behaviors and familial stressors.
Assist families in identifying internal resources and extended family support.
Develop and implement client rehabilitation services to enhance daily functioning.
Facilitate interagency and intra-agency communication, coordination, and referrals.
Attend community meetings and represent the agency at resource fairs.
Follow up with families who discontinue treatment to offer continued support.
Provide services in diverse settings including homes, schools, and communities.
Participate in training sessions and meetings as required.
Qualifications:
1-2 years of experience in case management with low-income and at-risk populations.
Bachelor's degree in social work, human services, or related field from an accredited institution.
