Location: Leimert Park, Los Angeles (Field-based, Home, School, and Community Settings)
Pay: $23.00 - $25.44 per hour, based on experience
Hours: Full-time, flexible schedule with potential for evening/weekend hours


Job Summary:
Our leading nonprofit client is seeking Case Managers to support children’s emotional well-being through various services. Case Managers will work with children, high-risk teens, and parents, conducting groups focused on building healthy relationships and promoting positive parenting. This role takes a strengths-based approach to empower families and ensures alignment with the nonprofit’s mission, vision, and core values.

Key Responsibilities:

  • Conduct needs assessments with children, youth, adults, and families to help them access resources like health insurance, housing, and childcare.
  • Provide individual and group rehabilitation services under agency guidelines.
  • Meet client service hour standards and submit progress notes within 24 hours using the electronic health system (EXYM).
  • Assist parents with managing child behavior and family stress.
  • Help families identify internal support resources, such as extended family.
  • Develop rehabilitation services to improve daily functioning at home, school, and in the community, in collaboration with the treatment team (therapist, psychiatrist).
  • Facilitate communication and referrals to other services when needed.
  • Follow up with families who drop out of treatment.
  • Represent the agency at community meetings and resource fairs.
  • Provide services in various community settings (homes, schools).
  • Participate in trainings and meetings as required.
  • Perform other duties as needed.

Required Qualifications:

  • 1-2 years of case management experience, ideally with at-risk populations.
  • Bachelor’s degree in social work, psychology, or a related field.
  • Valid CA driver’s license and an insurable driving record.
  • Use of a personal vehicle required for job duties.
  • Must pass a TB test and fingerprint clearance from the Department of Justice.
  • Proof of COVID-19 vaccination (or approved exemption) by start date.
