Job Summary:
Our leading nonprofit client is seeking Case Managers to support children’s emotional well-being through various services. Case Managers will work with children, high-risk teens, and parents, conducting groups focused on building healthy relationships and promoting positive parenting. This role takes a strengths-based approach to empower families and ensures alignment with the nonprofit’s mission, vision, and core values.
Key Responsibilities:
Conduct needs assessments with children, youth, adults, and families to help them access resources like health insurance, housing, and childcare.
Provide individual and group rehabilitation services under agency guidelines.
Meet client service hour standards and submit progress notes within 24 hours using the electronic health system (EXYM).
Assist parents with managing child behavior and family stress.
Help families identify internal support resources, such as extended family.
Develop rehabilitation services to improve daily functioning at home, school, and in the community, in collaboration with the treatment team (therapist, psychiatrist).
Facilitate communication and referrals to other services when needed.
Follow up with families who drop out of treatment.
Represent the agency at community meetings and resource fairs.
Provide services in various community settings (homes, schools).
Participate in trainings and meetings as required.
Perform other duties as needed.
Required Qualifications:
1-2 years of case management experience, ideally with at-risk populations.
Bachelor’s degree in social work, psychology, or a related field.
Valid CA driver’s license and an insurable driving record.
Use of a personal vehicle required for job duties.
Must pass a TB test and fingerprint clearance from the Department of Justice.
Proof of COVID-19 vaccination (or approved exemption) by start date.
