Full-Time (40 hours per week), Non-Exempt position. This is a contract-to-hire position! 
Hourly Compensation: $30/hr

Our leading nonprofit client is currently seeking a Case Manager to join their Whole Person Care team (also known as Full Service Partnership, Wrap-Around Services, or Assertive Community Treatment). This is a full-time, non-exempt position that reports to the Lead Case Manager. The role is based in San Rafael, CA, with work both in the office and in the field across Marin County, CA. Position Description:

Location: San Rafael, Marin County, Northern CA
Work Schedule: 50% on-site and 50% in the field

As a Whole Person Care Housing First Case Manager, you will provide flexible support services to individuals in need. This includes assisting clients with securing housing and coordinating healthcare. You'll work closely with other agencies to help clients access services like mental health care, vocational training, and entitlement programs (such as Medi-Cal and SSI).

Key Responsibilities:

  • Work with individuals and families who are chronically homeless and may have severe mental illness, physical disabilities, substance use history, or terminal illness.
  • Use clinical skills to assess client needs and determine eligibility for services.
  • Conduct mental health assessments and make medical referrals when necessary.
  • Prepare reports and case notes for WPC reimbursement.
  • Develop comprehensive care plans for each client and enter them into the WPC database.
  • Engage with clients at least three times a month to provide in-person support.

Qualifications:

  • Education: Bachelor’s degree required (Master’s degree preferred). Equivalent experience in mental health, social services, or a related field may be considered.
  • Experience: At least three years of experience in mental health, social services, or community services.
  • Skills:
  • Knowledge of Housing First, Harm Reduction, and Motivational Interviewing techniques.
  • Familiarity with social service programs and public assistance in California.
  • Strong understanding of social work practices, case notes, and record-keeping.
  • Ability to work with clients from diverse backgrounds, including those with physical or emotional challenges.
  • Other Requirements:
  • Ability to produce a negative TB test before starting employment.
  • Full COVID-19 vaccination (unless applying for an exemption) per state guidelines for healthcare providers.
  • Valid California Class C driver's license with a satisfactory driving record, as personal transportation is required for client visits.
