Our programs are designed to allow the individual to smoothly transition through every phase of their treatment plan.

As a Case Manager you'll be providing counseling and evaluating the effectiveness of interventions. Furthermore, you’ll be assessing the need for further resources and communicating with other members of the team regarding patient care.

Key Case Manager Responsibilities:

• Implementing the Plan of Care, providing counseling, and evaluating the effectiveness of interventions.
• Evaluating the need for further resources and assisting in accessing community resources.
• Communicating with other members of the team regarding patient care needs and any change in condition.

Key Case Manager Qualifications:

• Required: A valid CADC1 or above Credential.
• Preferred: At least one year of Case manager experience as a CADC1 within the last 3 years.
• Knowledge of and experience with addiction treatment techniques with adults and groups.

Final salary is per employer discretion and commensurate with experience.
